Illinois, 60451! Job Overview: Job title: Registered Nurse (RN) -
Home Health Location: New Lenox, IL Start Date: March 16, 2026
Duration: 13 weeks with possible extension Shift: Days (8:00 AM –
5:30 PM, 40-hour guarantee) Responsibilities: Provide patient care
through the nursing process (assessment, planning, implementation,
evaluation). Supervise CNAs and coordinate with other healthcare
workers. Conduct comprehensive initial and follow-up assessments
(physical, psychosocial, environmental). Initiate and update Plans
of Care; establish discharge plans. Complete OASIS documentation.
Educate patients, families, and caregivers. Ensure compliance with
federal, state, and agency regulations. Promote safety, privacy,
and confidentiality. Perform telehealth/virtual visits as needed. 5
