Job Description Participates in provider on-call rotation as required. Minimum of 1 year experience working with the frail or elderly population is required. Not accepting visa candidates Provider must be interested in Perm (not accepting Locums) ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE. Board Certified in Internal Medicine or Family Medicine - REQUIRED Board Certified in Geriatrics (Fellowship-trained) or Certificate of Added Qualifications in Geriatric Medicine (to be completed prior to start) - HIGHLY PREFERRED IN license (or willing to obtain) - REQUIRED Clean Malpractice - HIGHLY PREFERRED CDS Certification (by time of start) - REQUIRED DEA Verbal Confirmation (by time of start) - REQUIRED BLS (by time of start) - REQUIRED Minimum of 1 year experience working with the frail or elderly population (add to "Experience" field) - REQUIRED Previous supervisory experience (add to "Experience" field) - REQUIRED Does the provider require visa support - J1 or H1B?
