Job Description:

Interim HealthCare is seeking a dedicated Registered Nurse (RN) to join our team of compassionate healthcare professionals as a Start of Care (SOC) Nurse in our Home Health division. In this vital role, you will be the first point of contact for our patients beginning their home health care journey. You will assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Our focus is on providing patient-centered services tailored to individual needs, ensuring the highest level of care in the comfort and privacy of patients? homes. Conduct initial patient assessments to determine home care needs and develop individualized care plans. Initiate patient care meetings with interdisciplinary teams to coordinate and enhance care delivery. Educate patients and families on disease processes, medication management, and other pertinent health concerns. Document patient care services by charting in patient and department records according to accepted regulatory and company guidelines. Monitor and adjust care plans based on patient progression and input from the care team. Ensure compliance with all federal, state, and local regulations as well as company policies and procedures. Proficient in clinical assessments and ability to determine patient care requirements. Strong interpersonal and communication skills essential for educating patients and families. Ability to manage time effectively and respond flexibly to patient and team needs. Proficiency in using healthcare software and electronic patient records.
