qualifications):What is PACE?PACE, or Program of All-Inclusive Care
for the Elderly, is an alternative care model for those over 55
years of age requiring nursing home level of care which aims to
keep participants living at home in the community. PACE is a
collaborative, interdisciplinary approach which coordinates all
levels of medical and social needs, including prevention, wellness,
socialization, primary care, and access to specialists and other
services.The Advanced Practice Registered Nurse provides primary
medical care services to older adult (> 55 years of age) Program
of All Inclusive Care for the Elderly (PACE) participants using
primary care concepts, emphasizing prevention of illness and the
promotion of healthy lifestyles. Provides leadership and training
to other health care professionals. Functions as a member of the
PACE Interdisciplinary Team (IDT) in care coordination of PACE
participants; participates in the program planning of health
activities within the health Center and in the community, as
requested. Primary Duties and Responsibilities:Manages primary care
medical practice in PACE Center and in participants' homes, as
needed, by eliciting patient histories, performing physical
examinations, providing accurate clinical diagnoses, prescribing
evidence-based and cost-effective treatments, and providing patient
education.Maintains and promotes patient satisfaction through
acknowledgement that environmental, social and cultural factors
influence one's health including using AIDET in all patient
interactions.Participates in Quality Improvement activities of the
PACE center including peer review, and other designated
projects.Practices evidence based, cost-effective primary care in a
team approach model.Practices sound utilization management of
participant care within the PACE model.Makes appropriate diagnoses
and writes prescriptions and referrals consistent with Esperanza
Health Centers medical practice guidelines.Prepares, implements and
documents age-specific preventative health schedules for
patients.Maintains timely communication with partner hospitals,
specialists, nursing homes, and other contracted providers
regarding participants' services, to ensure continuity of care and
maintain oversight of care offerings provided.Maintains clinical
competency through regular continuing medical education and
provides documentation of such to Administration.Maintains liaison
with the partner hospitals' medical staff and ambulatory care
departments through obtaining and maintaining Hospital privileges,
attendance at department and section meetings and attending
physician teaching duties, as required. Takes call for Esperanza
Health Centers PACE participants as assigned by the PACE Medical
Director.Plays a role in educating and training medical and
healthcare professionals, as needed, and accepts and implements
teaching assignments of health center staff as assigned by the PACE
Medical Director. Requirements:Live up to Esperanza's mission of
being truly involved in the community all staff members are
expected to participate in outreach activities between 3 to 5 times
per year; including but not limited to the Annual Fundraiser,
Community Garden(s), and Health Fairs.Bilingual preferred.Must be
certified in CPR/BLS (Basic Life Support) by the American Heart
Association.Must possess and provide annually valid driver's
license and documentation of current automobile insurance at
agency-specified levels.Must provide negative TB certificate, or
documentation to support exemption, annually.Must comply with
requirements of other facilities (hospitals, LTC (Long Term Care)
facilities, etc.) related to flu or other vaccines if job duties
require providing services in those facilities. Graduate of an
Accredited nursing program. Graduate degree or post-master
certificate required along with completion of accredited Nurse
Practitioner Program Controlled Substance License and DEA.3 years
of experience with at least 2 years working with older adults
strongly preferred. Current IL State License, Board Certified or
Eligible in your Primary Care specialty.Licensed as an RN and APN
in the State of Illinois required; specialty in gerontology highly
